At your first appointment you will receive:

-Education on self-administering your doses at home, with the first dose administered  in the office. 

-Three syringes of your once-weekly medications, prefilled with your indicated dose of medication, to be kept refrigerated until use.

You will give yourself the injections at home once weekly. Every 4-week block you will receive a B12 injection (recommended), weigh in, review of symptoms, and your following month’s supply of medication, all included in our pricing.

If you have the medication sent directly to your home, keep in mind you must dose the medication yourself in syringes provided.

* Home delivery by clinical staff of your prefilled medication is available for a $50 surcharge. Injections can be provided by our staff in the office for $25 (excluding pick up and education day, which is included).

Possible side effects include but are not limited to:

Nausea, diarrhea, vomiting, constipation, abdominal pain, headache, fatigue, dyspepsia, dizziness, abdominal distension, belching, hypoglycemia, flatulence, gastroenteritis, and gastroesophageal reflux disease. 

Rare but serious complications include but are not limited to:

Thyroid C-cell tumor (animal studies), Medullary thyroid cancer, Hypersensitivity Reaction, Anaphylaxis, Angioedema, Acute Kidney Injury, Chronic Renal Failure Exacerbation, Pancreatitis, Cholelithiasis, Cholecystitis, Syncope, temporary or permanent Gastric Outlet Delay. Other possible long term effects are unknown.  

Contraindications include history of medullary thyroid cancer, multiple-endocrine neoplasia (MEN)-2, pancreatitis or if you are or plan to become pregnant.

You must provide or obtain a Comprehensive Metabolic Panel (CMP) within the last 12 months prior to starting therapy. This will need to be monitored every six months to ensure your health. You may obtain this lab through your primary care physician, or we can order the test through LabCorp and you can walk in any location to obtain your blood work without an appointment.

Your monthly treatment plan includes a complimentary physician consultation, all necessary lab work, close monthly follow up with our medical staff, monthly weigh-ins, a 1000mcg Vitamin B12 injection to keep you balanced, and access to our physicians to ensure safe and effective treatment. If you receive home delivery, you will receive your Vitamin B12 injection with your syringes.

Mail order Semaglutide and Tirzepatide will come directly from the pharmacy to your home.

Mid-month you will receive an email or phone call from our staff checking in on your progress. At that time you can change how you would like to receive your medication or schedule your office visit to receive your pre-filled syringes, check in and Vitamin B12 booster injection.

It is standard medical practice that ALL medications from a multi-dose vial (any vial that is punctured more than once) MUST be discarded after 30 days due to risk of medication degradation and bacterial infection. Any prefilled syringes from Initium must be discarded at 30 days and any vials from the pharmacy must be discarded 30 days after your first dose is withdrawn from the vial.
